Mentor Program Information
APPLYING TO THE MENTOR PROGRAM
Mentee
If a new and/or junior officer would like to be matched with
a mentor, please complete the mentee nomination form and send it to
LCDR Nicholette Y. Hemingway,
Chair of the HSO Mentoring Subcommittee.
All attempts will be made to match officers based on geographic
proximity, but this is not always possible. The major matching criterion
will be area of professional interest. Information about the HSO
Mentoring Program is included in the Welcome Letter sent by the
Health Services Chief Professional Officer and the Chair of the HS PAC.
Mentor
If a more senior officer would like to serve as a mentor,
please complete the mentor nomination form and send to
LCDR Nicholette Y. Hemingway.
Feedback
Program evaluation is critical to the improvement of the Mentoring
Program therefore Mentors and Mentee should fill out and submit
feedback forms within six months to one year of the initial Mentor-Mentee
match. The completed feedback form should be returned to the Mentoring
Subcommittee Chair.
