PHPAG Award Requirements

PHPAG Excellence in Leadership Award

The Public Health Professional Advisory Group (PHPAG) represents a diverse group of public health disciplines: Epidemiology, Generalist of Public Health, Health Promotion & Education, and Policy.  The PHPAG Excellence in Leadership award is presented to a Health Services Officer (HSO) who has demonstrated outstanding accomplishments in the field of public health, and is recognized by his/her peers for exceptional work toward the mission of the PHPAG.

Self-nominations will not be considered. The award will be presented at the annual USPHS Scientific and Training Symposium to be held in Glendale, AZ May 21-23, 2013. Applicants must meet Basic Readiness Standards in order to apply.

Nomination Package
Nomination Packets must include:
  • A Completed Nomination Form Nomination Cover Sheet
  • A narrative that answers the three questions listed in the Nomination form and is no longer than two pages.
  • Updated Curriculum Vitae.
  • Screen capture verifying basic readiness.
  • Additionally, nominees must have demonstrated outstanding accomplishments and has 3 or more years of public health experience in the U.S. Public Health Service.
